To schedule an appointment or learn more about our services, you can contact us by phone at 787-812-2525 or email us at theclinics@psm.edu.
Our service hours are from 8:00 AM to 5:00 PM.
We accept all commercial plans, MMM Vital and MMM Advantage.
Our clinic provides high-quality outpatient services, ranging from mental health to physical health care, tailored for the entire population.
The required documents for a minor to receive services are:

  • Health insurance card
  • Social Security number (last 4 digits)
  • Birth certificate
  • Vaccination certificate
  • Photo ID of the mother, father, or legal guardian
  • If the minor is under another custody arrangement, supporting documents or an affidavit are required.
