The purpose of this form is to provide Ponce Health Sciences University students with
a safe and confidential space to report any form of mistreatment they experience or
witness while participating in educational activities. Mistreatment, whether intentional
or accidental, occurs when behavior indicates a lack of respect for the dignity of
others and unjustifiably disrupts the learning process. Any form of mistreatment of
students will not be tolerated. This form is used to diligently investigate and take
appropriate action to address any abuse situation.Ponce Health Sciences University adapts and incorporates the AAMC model of Standards
of Conduct in the Teacher- Learner Relationship and encourages faculty and students
to familiarize themselves and pledge adherence to this policy statement:Preparation for a career in the healthcare profession demands the acquisition of a
large fund of knowledge and a host of special skills. It also demands the strengthening
of those virtues that are expected in the health provider/patient relationship and
that sustain the health profession as a moral enterprise. This policy statement serves
both as a pledge and as a reminder to teachers and learners that their conduct in
fulfilling their mutual obligations is the medium through which the profession inculcates

- Mistreatment reports are kept confidential, and their access is limited only to PHSU members who are necessary to reach a resolution process and or intervene in the situation.
- PHSU takes Mistreatment Reports very seriously.
- We strive to protect all students.
- It is possible to submit this form anonymously, although it is important to note that this restricts the ability to contact you and request additional information, which may hinder the resolution process.
- There are extraordinary situations, where it is required to disclose the identity of the person who sent this form. For example: the imminent threat of causing serious harm to students or other persons, criminal matters and/or mandatory Title IX reporting."
